Absence of a Consolidated Information Pertaining to Stroke
Stroke is one of the largest causes of adult physical disability in Singapore. There are existing resources, services and programmes to help stroke survivors but the information is scattered, residing mainly with individual service providers.
Solution
This Project Therefore Aims to Develop a One-Stop Community Portal
This portal serve as a community platform that aims to educate stroke survivors and their caregivers with practical advice and professionally endorsed best practices in caring for themselves and their loved ones. It will provide information on support services, resources that are available in the market and allows exchange of experiences and expert perspective on stroke management.
Outcomes
Stroke survivors and their caregivers will be equipped with professional advice in caring for themselves and their loved ones, gain access to information on support services and resources and build peer support networks.
